When you or someone you care about is at the end of life, Hospice care can turn a difficult period into a time to make final memories. Our hospice team provides care focused on symptom and pain relief, emotional and spiritual support and other services that can make all the difference for individuals and their family. It’s never easy to let go, but with the help of hospice caregivers and volunteers, it can be a way to transition from life, at home, surrounded by the people you love.
MC VNA hospice care typically involves your doctor, family members and caregivers as well as a hospice physician, nurses, social workers, spiritual care providers, therapists, home health aides and volunteers. We work closely with you and your family to meet your physical, emotional and spiritual needs and provide grief support. While services are provided primarily in your home, hospice care is available in other places including assisted living facilities and hospitals.
