VNA’s HIV/AIDS program focuses on caring for patients once
they have been diagnosed with this disease. These services are
made possible by a grant from the City of Detroit Department of
Health and Wellness Promotion, Grantee for the Ryan White Title
I funds through the U.S. Department of Health and Human Services.
Available services include:
Case Management—provides
case management to people living with HIV/AIDS. A case manager
helps the person living with HIV to manage his or her care by
putting them in touch with services available in the community,
keeping track of those services and providing advocacy and support.
They can also help locate a physician if the person does not already
have one. To access this service call (800) 882-5720 and ask for
the Intake Case Manager of the day. VNA has an after-hours component
(5 p.m. to 7 a.m. and weekend phone coverage) to respond to individuals
with urgent needs. They do not have to be case managed by VNA
to use this service.
Client Advocacy– provides brief
services to people living with HIV/AIDS who are in need of information,
service referrals, benefit assistance, and education but are not
appropriate for or do not want case management.
In Home Mental Health—provides
Ryan White funded in-home mental health counseling for HIV positive
individuals to assist them with depression and anxiety frequently
associated with HIV and issues related to coping with HIV diagnosis.
Services consist of a psychiatric evaluation, psychiatric medication
management if necessary, and therapy provided by psychiatric nurses
and social workers. To access this service call (800) 882-5720,
8 a.m. to 5 p.m., Monday through Friday, and ask for the Case
Manager of the day or Central Intake at (800) 852-1232.
Home Health Care—provides Ryan
White funded skilled nursing care for HIV positive individuals
to assist them with wound care, nutrition and diet, disease process
and management, skin care, physical therapy and occupational therapy.
Unskilled nursing care is available utilizing home health aides
to assist individuals with personal care and some basic housekeeping
needs. Persons wanting to access this service need to have their
medical provider call Central Intake during business hours, 8
a.m. to 5 p.m., at (800) 852-1232 or have them fax a referral
to (800) 852-8741. Requires at least a 7 to 10 day notice to schedule
a home health aide.
Treatment Adherence – provides
Ryan White funded Direct Observation Therapy (DOT) and/or weekly
medication set up inclusive of assessing, teaching, and monitoring
regarding medication, side effects and adherence. This service
is ordered by and coordinated with the patient’s HIV primary
care physician to address medication management and adherence
issues that the patient is experiencing. Persons wanting to access
this service need to have their medical provider call and ask
for the Case Manager of the day or Central Intake at during business
hours, 8 a.m. to 5 p.m., at (800) 852-1232 or have them fax a
referral to (248) 967-8741.
Primary Care—
individuals who have no insurance coverage or are low-income with
high private insurance co-pays Ryan White funded primary care
